Step 1: Complete this brief contact form.

Step 2: Erickson Financial Services will contact you to discuss the situation.

Step 3 (For clients who have already had diagnostic tests): If you decide to move forward, Erickson Financial Services will send any test results and/or medical records that have already been done to the potential healthcare provider. There is no charge for this service. If the potential healthcare provider determines that you are a good candidate for surgery based upon the information that we send, Erickson Financial Services will provide you with a price quotation and the name of the healthcare provider that can best treat your healthcare situation. At this time, a consultation appointment will be scheduled.

Step 3: (For clients who have not had diagnostics yet): Based upon your symptoms, we will suggest a likely treatment plan. An appointment will be made for diagnostics, and you will travel to the facility. There is a $200 consultation fee that will need to be paid directly to the surgeon. After the diagnostics are completed, an estimate for the course of treatment will be provided. If this is acceptable to you, a face-to-face consultation with the surgeon will be arranged.

Step 5: Following the consultation, if you determine that you want to go ahead with treatment, your surgery will be scheduled.

Step 6: Prior to surgery, you will send payment in full (minus the $200 consultation fee), in the form of a certified check or cashier's check, to Erickson Financial Services (The check will be made out to Erickson Financial Services). Once payment is received, your surgery will be scheduled.
