Blue Preferred
Blue Preferred is Anthem's "Top of the Line", full coverage healthplan. This health insurance plan offers the most similar benefits as those one might recieve via an employer sponsored healthplan. However, this plan does not offer a maximum family out of pocket, so it is not ideal for large families. Benefits include:

A low office visit copay of $25.

A prescription drug card with copays of $15 for generics, $45 for brand name, and $60 for non-preferred brand name drugs. Other than for diabetes, self-injectable drugs are not covered on this healthplan.

Adult preventive care:

One annual pap test: $25 copayment for office visit and $75 maximum payment for laboratory test.

Mammogram screening and prostate screening, which are not subject to deductible or coinsurance.

Child preventive care is covered at 80%, and it is not subject to the plan deductible.

First dollar accident coverage: $500 first dollar coverage per person/per accident.

The per person lifetime maximum benefit on the Blue Preferred Plan is $2 million.

Right Plan PPO 40
The Right Plan PPO 40 is ideal for those wanting simple, immediate benefits with no medical deductible. You pay a simple, $40 copay for routine, doctor's office visits. This personal health plan is available to individuals only; not families. Benefits include:

$40 office visit copay.

Generic prescription drugs are covered under a $10 copay.

Brand name prescription drugs: $500 separate deductible, THEN a $30 copay for formulary drugs, a 50% coinsurance for non-formulary drugs, and a 30% coinsurance for self-administered injectables.

Tonik
Tonik health insurance is primarily marketed to healthy individuals between the ages of 19 and 29 years of age. To qualify for the Tonik plan, you must be in perfect health with no pre-existing or ongoing health conditions. Tonik's benefits include:

100% coverage after deductible.

ER coverage under a $100 copay.

Preventive dental and vision care.

Lumenos
The Anthem Lumenos Consumer-Driven and HSA compatible health plans provide coverage after an individual or family deductible is met. All services apply towards the plan deductible, at a discounted rate with in-network providers, until the plan deductible is met, at which time, coverage begins. The only services that are not subject to the plan deductible are preventive care services. Lumenos plans offer the most comprehensive preventive care coverage in the marketplace. Preventive Care is not subject to the plan deductible AND is covered at your chosen co-insurance level (80% or 100%). Preventive care includes:

The Anthem SmartSense plans are Anthem's newest product line, just released during the last quarter of 2008. SmartSense offers reliable, basic protection at Anthem's lowest rates. You choose the annual deductible/monthly rate combination that works for you. This plan offers:

A choice of prescription drug benefits (Generic Only or Comprehensive).

This healthplan is ideal for large families, because the family deductible can be satisfied by 2 or more family members, and the annual out of pocket maximum includes deductibles, coinsurance and copays.
