Patient Direct Plan
The Patient Direct Plan is Delta Dental's most affordable individual dental plan, starting at just $14.25/mo for an individual. This plan is a DISCOUNT dental plan. It is NOT insurance. The Colorado Delta Dental Patient Direct Plan provides the following benefits:
- Discounts of 50% to 100% on dental procedures
- 21%-23% percent savings on child and adult orthodontia
- No waiting periods for covered services
- No benefit year maximums or annual limits
- The member MUST receive care from a participating dentist or specialist.

PPO Direct Plan
The PPO Direct Plan is the most affordable "Delta Dental of Colorado" individual Dental Insurance Plan, starting at just $28.00/mo. The Delta Dental of Colorado PPO Direct Plan offers the following features and benefits:
- 1130 PPO Dentists (You MUST use a PPO Dentist to receive benefits on this plan)
- $1000 Benefit Year Maximum
- $75 Deductible (waived for preventive services)
- No waiting period for fillings!
- 100% coverage for preventive care
- 70% coverage for fillings
- 40% coverage for Type IIIA and Type IIIB major services
- 12 month waiting period for Type IIIA Major Services (credit given towards waiting period with proof of prior coverage**)
- 24 month waiting period for Tyype IIIB Major Services (credit given towards waiting period with proof of prior coverage**)
- There is no orthodontia coverage offered on this plan, but if you use a PPO Dentist, you will receive discounted rates for orthodontic services based on the PPO Dentist's allowable fee.

PPO Plus Direct Plan
The PPO Direct Plan is Delta Dental's top of the line individual Dental Insurance Plan, starting at $36.00/mo. The Delta Dental of Colorado PPO Plus Direct Plan offers the following features and benefits:
- 1130 PPO Dentists
- 2500 Delta Dental Premier Dentists (NOTE: coverage is reduced when a Premier Dentist is used. See Delta Dental Plan Comparison).
- You may use any non-network dentist. This plan will pay a percentage of the allowed amount for a PPO Dentist. Delta Dental Premier dentists and out-of-network dentists will balance bill you for excess charges. See Delta Dental Plan Comparison.
- $1500 Benefit Year Maximum
- $50 Deductible (waived for preventive services)
- No waiting period for fillings!
- 100% coverage for preventive care (90% when a Premier Dentist is used***)
- 80% coverage for fillings (70% when a Premier Dentist is used***)
- 60% coverage for Type IIIA and Type IIIB major services (50% when a Premier Dentist is used***)
- 12 month waiting period for Type IIIA Major Services (credit given towards waiting period with proof of prior coverage**)
- 24 month waiting period for Type IIIB Major Services (credit given towards waiting period with proof of prior coverage**)
- There is no orthodontia coverage offered on this plan, but if you use a PPO Dentist, you will receive a discounted rate for orthodontic services based on the PPO Dentist's allowable fee.

Important Information
**If you have been enrolled in a dental insurance plan within 90 days of your effective date on this plan, Delta Dental will reduce your waiting period based on the amount of time you had prior coverage. For consideration, submit your certificate of prior dental coverage before your effective date. If you do not have a certificate of prior dental coverage, Delta Dental will accept your certificate of prior dental coverage as many as 30 days after policy inception. Best practice is to remit the certificate to Delta Dental as soon as it is received. Fax your certificate with a cover letter of request to remove your waiting period to Delta Dental at 303-773-3880.
***If you choose to use a non-network Dentist or a Delta Dental Premier Dentist with the PPO Plus Direct Plan, your plan will pay a percentage of Delta Dental's PPO Dentist's Allowable Fee, and you will be responsible for any excess charges. If you choose to use a Delta Dental Premier Dentist, you will be responsible for any excess charges including the difference between the Premier Dentist's Maximum Allowable Fee and the PPO Dentist's Allowable Fee. For the best possible coverage, Erickson Financial Services recommends choosing a dentist from Delta Dental's PPO Network (as opposed to Delta Dental's Premier Network).
